AAIB investigation to Phoenix Wings Orca, (UAS reg n/a)

Inadvertent in-flight shutdown resulting in lack of management, Coombe Nation Park, Warwickshire, 25 March 2025.

The plane struck the bottom in a wooded space while on strategy to land at a website adjoining to Coventry Hospital. The accident occurred through the sixth consecutive flight, which was in preparation for demonstrating the plane being operated in accordance with the operator’s Past Visible Line Of Sight with Visible Mitigations (BVLOS VM) authorisation.

The reason for the accident was recognized as a software program bug together with a lack of synchronisation between the Distant Pilot (RP) and the Security Distant Pilot (SRP), whereby the SRP’s hand controller had remained set to the disarm place when the plane had taken off.
